Title 25 Employees
BIE Employees unable to complete the SMIS vaccination reporting requirements need to provide their Vaccination Specialist with a completed form DI-6510 (Attachment below) and corresponding vaccination documentation via email (fastest way), or fax to (505) 563-5329.
November 5, 2021 was the deadline for Title 25 employees to complete their vaccination reporting requirements in accordance with Secretarial Order No. 3402. Noncompliance may result in discipline, up to and including removal from the Federal Service.
Title 5 Employees
