Employment Verification Forms
Employee can fax employment verification forms to Human Resources Office at 505-563-5305 - Attention: Staffing
Employees can email forms via their Bureau of Indian Education email (BIE.edu) to the Human Resources Specialist that services their location. Contact your Business Office to identify who is your serving Human Resources Specialist.
We recommend that employees do not send Employment Verification Forms from a personal email address because of Personable Identifiable Information (PII) listed on the form and potential interception.
Employment verification forms MUST have the following:
- Employee Social Security Number
- Statement authorizing disclosure to third party
- Employee wet signature and date OR digital signature with PIV card
- Employee contact phone number
The Bureau of Indian Education Human Resources Office will contract the employee to validate identity and verify that requested data disclosure. Employees are responsible for being available otherwise delays can be expected.
The Bureau of Indian Education Human Resources Office will not fax or email completed employment verifications to third parties. Employees are responsible for submitting completed forms to appropriate party.

The Work Number is an online and phone-in automated service that provides a secure way to authorize requesters (such as prospective employers, social services agencies, lenders, insurers, and rental companies) to verify employment and wages. The Work Number is available 24 hours a day, seven days a week, and handles the employment verification requests for agency employees (with data provided by IBC).
Any time there is a need to respond to a request for employment or wage information. The Work Number is a simplified and efficient method used for handling employment or salary/wage information requests.
The best way to access the Work Number online is through Employee Express. Once you are logged in to Employee Express, go to Related Links and click the Work Number: Proof of Employment and Income which will forward you to the WorkNumber website to create Salary Key request.
